Why Bone Loss Happens and What It Has to Do With Implants

When you lose a tooth, the jawbone in that area loses its purpose. Tooth roots naturally stimulate the bone every time you chew. Without that root, the bone literally begins to melt away. Severe gum disease also destroys the bone structure from the outside in.

Dental implants act as artificial roots. They stop bone loss in its tracks and give you your bite back. However, these titanium posts need a solid foundation to hold them in place. If your jawbone has shrunk significantly, the implant will not have enough structure to anchor securely.

How Much Bone Is Actually Needed (and How That Is Measured)

To hold an implant securely, you need enough bone width and height to surround the post completely. Many general dentists evaluate bone using standard 2D X-rays. A 2D image simply cannot show the true three-dimensional shape of your jaw.

At Rubino Periodontics, we use 3D CT scanning to evaluate your mouth accurately. You can read more about this equipment on our advanced technology page. A 3D scan lets us see the exact millimeter of bone available. Often, what looks like “too little bone” on a flat X-ray is actually perfectly adequate when viewed in full 3D. This precision allows us to find healthy bone that a 2D X-ray misses.

What Bone Grafting Actually Involves

If your 3D scan confirms that your jawbone is too thin or short, we can rebuild it. Bone grafting & sinus lifts are standard procedures for a periodontist. During a graft, we place safe bone-building material into the deficient area. Your body uses this material as a scaffold to grow its own new, solid bone.

Bone grafting does add time to your treatment timeline. It typically takes a few months for the new bone to heal and harden enough to support an implant. This wait requires patience, but it creates a secure foundation that can last for decades.

Sinus Lifts: What They Are and When They Are Necessary

The upper jaw has its own specific anatomy. Just above your upper back teeth sit your sinus cavities. When you lose an upper tooth, the bone shrinks, and the sinus cavity often drops down into that space. If we place an implant there without enough bone height, it would push into the sinus membrane.

A sinus lift gently raises the sinus membrane back to its proper position. We then place bone grafting material underneath it. This creates the vertical height needed to secure an implant safely in the upper back jaw.

All-on-4: Bypassing Bone Loss Entirely

Sometimes, rebuilding bone across the entire jaw is unnecessary. If you are missing all your teeth on the top or bottom, we offer a remarkable solution called All-on-4. This approach uses just four strategically placed implants to support a full arch of permanent replacement teeth.

Instead of placing the posts straight down, we tilt the back implants at an angle. This angle allows us to anchor the posts into the dense bone at the front of your jaw, completely bypassing the areas in the back where bone loss is most severe.
